On October 12, 2022, OrthoAlaska, LLC discovered that its network had been subject to to unauthorized activity and began investigating. OrthoAlaska, LLC finished its investigation and determined that its employees and/or customers’ personal information may have been compromised, including:
- Name
- Financial account number
- Credit/debit card number with security code/PIN

- Keep a copy of the notice for your records; and
- Enroll in the identity theft and credit monitoring services offered in the notice from OrthoAlaska, LLC..
